The deadline for certifying the Form 471 and submitting the Item 21 Descriptions of Service Attachments for E-rate Funding Year 2011 is May 17th.
A Form 471, or the “Services Ordered and Certification Form,” must be filed by schools, libraries or their consortia to request discounts on eligible services. The Form 471 certifications for Funding Year 2011 must be received or postmarked on or before May 17, 2011 to be considered “in window”. Any Form 471 certified after the 17th will be considered “out of window” and will most likely not be funded in FY2011.
Each Form 471 Block 5 Discount Funding Request must include a description of the products or services for which the applicant is seeking discounts. These descriptions, more commonly referred to as Item 21 Descriptions of Service, also have a submission deadline of May 17th. If the Item 21 Descriptions of Service Attachments are not submitted before May 17th, the FRN associated with the missing Item 21 information will be canceled. Funding Year 2011 PIA will not start until the Items 21 are received.
