USAC has announced that they enabled online filing for Funding Year 2012 for the Form 470.
A Form 470 is an applicant form that identifies the “Description of Services Requested and Certification Form” and must be filed to seek competitive bids for products and services for E-Rate discounts. The 470 Form may be filed at any time during the year and must be posted on the SLD web site for at least 28 days prior to filing a Form 471.
USAC will also begin processing paper Forms 470 that feature Funding Year 2012 in Block 1.
If an applicant has already filed a Form 470 for FY2011 and noted in Item 13 that they intended the form for FY2012 services, they should remember that they cannot cite this form for non-contracted services provided under tariff or on a month-to-month basis. In these instances, an applicant must file a Form 470 featuring FY2012 in Block 1.
