On May 6th, 2014 the SLD notified applicants who have not yet certified their Form 471 and applicants who have yet to submit their Item 21 attachments. The SLD has given these applicants until May 27th, 2014 in order to complete the application process. If the applicants fail to meet the extended deadline, these Forms 471 will be deemed out-of-window and will not receive E-rate support for FY 2014.
